Purpose & Usage
tx.fhir.de is a FHIR R4 terminology server primarily used for FHIR tooling and validation. It enables ValueSet expansion, code validation, and terminology services according to HL7 FHIR R4.
This server is not intended for bulk download of terminology content. For downloads, please contact the respective sources (e.g., BfArM for ICD-10-GM, OPS).

Role of the ZTS
The Central Terminology Server (ZTS) is intended to serve as the authoritative reference source (“source of truth”) for all coding systems, value sets and mappings relevant to the German healthcare system. Terminologies for the German healthcare system are provided, versioned and published there. Other terminology servers may use this content as a reference and reuse it for their own purposes. Independent publication of this content by other servers is, however, not permitted.
Role of the HL7 terminology server
The HL7 terminology server is primarily intended to enable FHIR community tooling, such as the validation of German-specific terminologies or the use of the IG Publisher. It is not intended for direct use as a primary terminology server in production systems.
Unlike the Central Terminology Server (ZTS), which acts as the reference source for terminologies, the HL7 Deutschland terminology server is embedded exclusively in the international HL7 terminology ecosystem. It is not addressed directly. Instead, requests are made via the international HL7 terminology server, which – where necessary – forwards them within the ecosystem to the HL7 Deutschland terminology server for the processing of German-specific terminologies.
Interaction
Both servers address different use cases and each therefore has its place. The HL7 Deutschland terminology server will regularly obtain new or changed terminologies from the ZTS and hold them internally for further processing. In the longer term, the syndication feature is to be used for this purpose.
The terminologies are not offered for download on the HL7 Deutschland server. The server serves validation purposes only, i.e. checking the correct use of terminologies. To this end, the stored content is prepared accordingly (e.g. through expansions) and made available in the form of ValueSets.
Terms of use
The terms of use of the individual ZTS terminologies are respected in that they are not made available for download, but are processed internally for validation purposes only. A landing page also transparently points to the respective terms of use.
HL7 International Terminology Infrastructure
tx.fhir.de is integrated into the international HL7 FHIR terminology infrastructure, extending it with German terminology content.
